1. Data Collection & Intake Parameters

Quiet Horizons TelePsychiatry collects specific Protected Health Information (PHI) to evaluate your eligibility for our concierge practice. By submitting the intake form, you consent to the secure transmission of the following data points:

  • Identity & Contact: Legal Name, Date of Birth, Email, and Phone Number.
  • Jurisdiction: Proof of Massachusetts residency.
  • Clinical History: Previous psychiatric care history, current or past diagnoses, and active medication regimens (including dosages and frequencies).
  • Clinical Intent: The stated reason for your visit.

2. Cryptographic Safeguards & Data Siloing

Your data is not stored in plain text. Quiet Horizons utilizes an advanced, application-layer encryption protocol.

  • Encryption at Rest: All clinical data submitted through the digital front door is encrypted using AES-256-GCM standards before it is ever written to the database.
  • Immutable Auditing: Every instance where your medical record is accessed, read, or modified by the Medical Director triggers an automatic, immutable audit log to ensure total visibility and compliance with federal privacy laws.

3. Infrastructure Tracking & Cookies

Our digital infrastructure is designed to protect clinical privacy over marketing analytics.

  • Zero Persistent Tracking: The application explicitly forbids the use of localStorage for PHI. We do not use third-party advertising trackers or pixel retargeting.
  • Session Management: Any data entered into the intake form is held temporarily in volatile sessionStorage. If you abandon the form or close your browser, the data is destroyed.
  • Provider Safeguards: Clinical access portals are protected by strict 15-minute inactivity timeouts, terminating sessions automatically to prevent unauthorized physical access to your dossier.

4. Terms of Service Limitations

By utilizing the digital intake infrastructure, you agree to the following operational parameters:

  • Age & Jurisdiction Restrictions: The system programmatically rejects intake submissions from individuals under the age of 18 or individuals residing outside the state of Massachusetts.
  • Triage Nature: The submission of an intake form does not establish a formal doctor-patient relationship. Intake data is retained for administrative triage. If you are not accepted into the practice, your data remains secured and encrypted under standard HIPAA retention requirements (minimum 6 years) but is not actively processed.
  • Emergency Limitations: This platform is not an emergency response system. If you are experiencing a medical or psychiatric emergency, dial 911 or proceed to the nearest emergency department.
